Bogus Jury Form Linked to Identity Theft: Cops

Plainfield resident provided her Social Security number, not realizing the questionnaire she filled out was a scam, police say.

A Plainfield resident who had her identity stolen after completing a bogus Kendall County Jury Commission questionnaire has prompted Kendall County to issue a warning about the scam.

The resident, who lives in the Kendall County portion of , received the juror questionnaire on Oct. 28. It asked that personal information be provided, including a Social Security number, according to a news release issued by the Kendall County Sheriff's Office.

The respondent complied and later became a victim of identity theft, the release said. When shown a copy of the actual jury commission questionnaire, the victim was able to verify that the form she completed was different, the release said.
